Initial Consultation, Care Plan, and 1-Hour Follow-Up Visit

This includes a 2-hour consultation, emailed care plan, and a 1-hour care plan follow-up visit.

Initial Visit: During the initial two-hour consultation, Amy will meet with family members online to comprehensively understand your family's unique situation. This session will also serve as an educational opportunity, where Amy will provide vital information to help you navigate the complexities of dementia. Following the consultation, Amy will formulate a personalized care plan tailored to your needs. The care plan will include:

• Insight about your family's dementia journey.

• Guidance on communication and support strategies.

• Safety recommendations for the physical and social environments.

• Guidance for respite care, including home care support, VA services, long-term care insurance, Medicaid considerations, and care facility options.

• Guidance for guardianship, conservatorship, power of attorney (POA), and advance directive documents.

Care Plan & Follow-Up Visit: Following our initial consultation, you will receive an email with your family's care plan. We will discuss the care plan, your progress with any of your important goals, and any questions you may have during the follow-up visit.
